Students who wish to request academic accommodations, must complete the Accessibility Registration Form the first time they request accommodations or if it has been more than five years since they have accessed our services.

Required Information

We recommend that students review the information below to help guide you through the process. Make sure to have your supporting documentation ready to submit as PDF, PNG or JPG in the application. You will not be able to submit the application without documentation.

Before you get started, please locate the following information needed to complete the request form:  

  • College email address (xxxx.xxx###@keyanomail.ca) 
  • College Student ID number (000-###-###) 
  • PDF version/JPG of your medical documentation. Please include your last name and student ID number in the file name (smith-000-###-###.pdf)  

 

How to Register

 Step 1: Collect your supporting documentation and use appropriate file name

Accepted Supporting Disability Documentation Requirements:

  • Either be in English or have attached to the original documentation a certified English translation;
  • Confirm the rationale for seeking accommodation;
  • Be from a credentialed health professional;
  • Include the credentialed health professional’s name, title, and contact information;
  • It must be stamped with the professional’s stamp or be on official letterhead;
  • Include the date of the assessment;
  • Include a statement on the nature of the disability and the care plan including any medications and the impact of not following the care plan;
  • Explain the impact of the disability on the student in a post-secondary learning environment; a diagnosis alone is insufficient.

 

If you are needing Schedule IV Funding, it is suggested that you have a qualified healthcare professional complete the Verification of Disability Form:

Even if you are not applying for this funding, we will accept this completed form as supporting documentation.

Step 2: Students who are seeking accommodations for the first time must complete the Accessibility Registration Form
  • You will need your Keyano College Student ID. 
  • You will need supporting documentation. 
  • You will need your course codes and instructor names, if known. 
  • You are not approved for academic accommodations until you receive an email approval from the Accessibility Services office.  
  • All communication will be received through college e-mail. 
  • NOTE: This form only needs to be completed once within 5 years. After that, accommodations will be renewed using the Accommodations Renewal Form.
 Step 3: Check your Keyano College email account frequently for communication from our office
  •  Accessibility Services will contact you within 10 business days via email.
  • If your requested accommodations are approved, accommodation letters will be sent to your instructors, and you will be cc’d. You will also receive an email from an Access Strategist with important information relating to Testing Services.
  • If any further information is needed based on your registration form, and/or we require you to book an appointment with an Access Strategist to go over more details, you will receive an email notification.     
Step 4: If a meeting is requested from the Accessibility Services office, you will be required to attend an appointment with an Access Strategist

During this meeting, an Access Strategist will: 

  • Ask any further questions from the information provided in your completed form.  
  • Discuss and approve your academic accommodations.  
  • Provide information about our services at the college. 
  • Discuss your responsibilities and when to contact an Access Strategist for assistance.  
  • Answer any questions you may have about your accommodations.  
 Step 5: Complete any follow-up appointments

If your approved accommodations include adaptive technology or if you wish to get help with Schedule IV Funding applications, you must book an appointment with our Adaptive Technologist.

 Step 6: Renew your academic accommodations at the beginning of each term
  • To Renew your accommodations each term, you must submit the Accommodations Renewal Form. 
  • We will process your accommodations renewal application based on the accommodations you've had most recently and advise when they have been approved. 
  • Note: Processing time may take up to 10 business days.

 Step 7: Contact Accessibility Services if:
  • You encounter any problems with your accommodations, have new barriers or a new diagnosis.  
  • At least four months in advance of learning placements (co-op placements, practicums, etc.) to ensure adequate time to implement an accommodations plan.
